  • Course Review Form (Also used for new course proposals): Faculty liaisons should complete this form when a high school requests to offer a CITS course not previously offered at their school, when a high school teacher alters their existing course syllabus, when the LSC course outline has changed, or when a new mentor or new teacher is assigned to a course. A complete and thorough review of the course, the syllabus, and course assignments should accompany the form. Complete the form fully, send a copy to the high school teacher, and upload to D2L.   • Site Visit Form: Faculty liaisons should complete this form for each course and each visit. Complete the form fully and upload to D2L. See page 7 of the College in the Schools Faculty Handbook for more details.
  • Professional Development Tracking Form: Use this form to document professional development activities completed with your mentees. Submit completed forms and any additional documentation to the CITS Director.

Travel and Reimbursement Forms and Links

  • CITS Travel Process: See page 8 of the Faculty Liaison Handbook below for more details.
  • CITS Online Leave Form: Faculty liaisons should submit this online form prior to each visit. Submission of this form will notify the CITS Director and your academic dean that you plan to travel on college business.
  • Expense Reimbursement Form: Complete this form after travel. Submit to the CITS Director with an email confirmation of your visit and a map printout.
